This annual event offers continuing education, practice information, and legislative updates for HHAC members and other Home Care, Home Health and Hospice professionals in the Rocky Mountain region. Education sessions are designed to be applicable to all sectors and levels of the profession and will provide the latest techniques and trends as well as basic skills.

The 2021 HHAC Pre-Conference will be on-demand this year & registration is separate from the main conference. The pre-conference sessions will be available online on Sunday, October 10, 2021 to watch on-demand and receive continuing competency credits, and will be available through January 31, 2022. Pre-Conference Session #1
Title: Focus on Reducing Acute Care Hospitalizations

Acute Care Hospitalizations is a key measure that impacts your Quality of Care Star rating and your Total Performance Score in Value-Based Purchasing, proposed to include all states in 2022. The Conditions of Participation require both identification of risk for hospitalization and implementation of interventions to address those risk factors (resulting in a frequent survey deficiency citation!). This session examines the forces that drive re-hospitalizations of home care patients as the first step to reducing admissions, then provides evidence-based best practices and outlines a QAPI Performance Improvement Plan to address this important quality measure. Bonus: take home tools and best practices to reduce ACH.

Title: Survey Readiness is NOT Optional: What Can Providers Do in 2021?

With the home health and hospice industry under increased regulatory scrutiny, there has been a significant increase in survey deficiencies and focused survey challenges for both home health and hospice. This program will provide an overview of most problematic survey issues impacting home health and hospice providers in 2020-2021 and identify strategies and resources home health and hospice providers can use to establish an ongoing survey readiness culture and process within their organizations.

Attendees can receive up to 12 hours of credit during the conference towards their state licensing requirement. Additional details on continuing education for State Licensure, Nursing and PT can be found here.
HHAC is working closely with the Keystone Conference center and local health authorities as we monitor the ongoing COVID-19 situation. Starting September 7th, the Keystone Lodge & Spa and Keystone Conference Center will be implementing a mask mandate for public indoor spaces and has not issued guidance about social distancing. As a result, we are: (1) requiring masks or face coverings that comply with government guidelines and mandates in our meeting spaces as well as all public hotel spaces; (2) keeping the desire for social distancing in mind as we consider room layouts; (3) continuing to stay in close touch with the hotel since this is such a dynamic situation . All decisions that we make are done so with the safety and health of our members and conference attendees as our top priority. As the situation related to COVID-19 changes, we will work in concert with hotel and local government officials to make decisions that will keep everyone safe. We will update the website and send email updates as we receive any new information.
CONFERENCE CANCELLATION POLICY:
All cancellations and requests for refunds must be submitted in writing to HHAC. Refund requests received by Monday, September 13 will receive a full refund minus a $25 administrative fee. Refunds received from September 14 through October 4 will receive a 50% refund minus a $25 administrative fee. There are no refunds after October 4 or for no-shows. Refunds will be processed 30 days after the event. Substitutions are encouraged. By attending this conference, permission and license is given to publish, reproduce, tape, print, distribute, and/or otherwise use my name and any still or moving photographic image, likeness or sound recordings.
